The Philip Jaisohn Memorial Foundation (Jaisohn Center) has opened the Jaisohn Community Wellness Plaza, a new one-stop community hub designed to bring health, wellness, and human services together under one roof for seniors and families.
Built on 50 years of service to the community, the Jaisohn Community Wellness Plaza was intentionally designed to strengthen continuity of care, improve access to services, and support older adults in aging with dignity in their own community. By integrating services in one trusted location, the Plaza reduces barriers to care and creates a more coordinated, welcoming experience for patients and families.
Today, the Plaza offers primary care, home health care, home care, mental health counseling, health insurance assistance, community health navigation, and social services. In the future, Jaisohn plans to expand services to include blood work, acupuncture, pharmacy, pain management, as well as an ophthalmologist and dermatologist.
“The Jaisohn Community Wellness Plaza represents an important milestone in how we serve our community,” said George Choe, president of the Jaisohn Center. “By bringing health, wellness, and human services together in one place, we are strengthening continuity of care and creating a space that reflects our commitment to dignity, accessibility, and trust.”
The new Plaza emphasizes ease of navigation, privacy, and accessibility, with a layout designed to be welcoming—particularly for older adults and individuals accessing services for the first time. The space also reflects Jaisohn’s longstanding commitment to culturally and linguistically responsive care, ensuring services are delivered in ways that respect cultural values and lived experience.
Located at 1180 Welsh Road in Upper Gwynedd Township, in the former Gwynedd Corporate Center, across from Assi Plaza, the new Jaisohn Community Wellness Plaza is a transformative investment that serves as a central point of connection for individuals and families seeking coordinated, community-centered support.
